We received a letter stating that Anthem had received information from the IRS that their records didnt match. We needed to complete a form which included my name and birthdate it required I add my SS number, sign and date it. This packet was incredibly professional, including all the usual inserts, using the standard Anthem font, etc.
After speaking with Anthem, including their Fraud team, this is a FAKE. They stated they will NEVER request a SS number in a printed form this kind of thing would be dealt with through our employer.
I dont know if this scam is circulating among other insurance companies or if its just our insurer or our state. BUT IF YOU RECEIVE A LETTER FROM YOUR INSURANCE COMPANY REQUESTING YOU FILL IN YOUR SS NUMBER, ASK QUESTIONS BEFORE YOU COMPLY.

To the best of my knowledge they aren't even my insurer, so I ignored it. It hasn't been repeated, and I have not received any follow-up.
